EX-17 — EX-17 — TIE Application Form

The official application form for the Foreigner Identity Card (TIE). Used for the first issuance, renewal, and replacement of the TIE card.

How to get it

Download the PDF from the Spanish Ministry of Inclusion website. Fill it out digitally before printing, or print and fill by hand in BLACK ink only.

How to fill it

Mark the box at the top for the reason (initial issuance, renewal, etc.). Use the exact name and date of birth as they appear in your passport. The address must be your registered (empadronado) address in Spain.

Field-by-field guide

Before you start

Print the form. Fill it on PC or by hand in BLACK ink. Hand it in at your police appointment. No digital certificate needed.

1) DATOS DEL EXTRANJERO/A — Your info

Copy everything exactly from your passport.

PASAPORTE: Passport number, no spaces.
Example: AB1234567
N.I.E.: Find this on your student visa page in your passport — it's the long number labeled NIE.
Example: Y1234567X
1er Apellido: First surname from passport.
Example: GARCIA
2º Apellido: Second surname. Blank if you don't have one.
Example: LOPEZ
Nombre: First name(s) from passport.
Example: MARIA ISABEL
Sexo: H = male, M = female, X = non-binary.
Fecha de nacimiento: Day/Month/Year.
Example: 07/03/2002
Lugar / País: City and country of birth.
Example:

2) DATOS DEL REPRESENTANTE

Leave blank. Only filled by a lawyer or gestor filing for you.

3) DOMICILIO A EFECTOS DE NOTIFICACIONES

Address for official mail — usually copy Section 1.

CONSIENTO checkbox

Skip — leave UNCHECKED. It's only for digital filing.

4) Reason — next page

Tick the box for first issuance (the one labeled INICIAL). Sign + date at the bottom in BLACK ink.

Common mistakes

— Filling the form in blue ink — must be black
— Using a nickname or anglicized name instead of the exact passport name
— Listing your home country address instead of your Spanish address
— Forgetting to sign at the bottom — unsigned forms are rejected at the counter
— Printing only one copy — bring two printed copies (one for them, one stamped for you)

Always download a fresh copy on the day you fill it — the form is updated periodically and old versions are sometimes rejected.
